Fistula in Ano
An abnormal tunnel connecting the anal canal to the skin around the anus, almost always the sequel to a perianal abscess. It will not close on its own.
Symptoms
Recurrent perianal swelling, an intermittent discharge of pus or blood-tinged fluid staining the underclothes, itching, and a persistent small opening on the skin near the anus.
Recent advances in management
Laser fistula ablation (FiLaC) is now the gold standard for simple and intersphincteric fistulae — a laser fibre is passed along the tract to obliterate it from within, sparing the sphincter and protecting continence. Sphincter-sparing LIFT and video-assisted (VAAFT) techniques handle the more complex tracts.
