Intestinal Obstructions
Blockage of the small or large bowel — mechanical (adhesions, hernias, tumours) or functional (paralytic ileus). A surgical emergency that requires prompt diagnosis and, often, prompt operation.
Symptoms
Colicky abdominal pain, progressive distension, bilious or faeculent vomiting, absolute constipation with no passage of stool or flatus, and — as it progresses — dehydration and fever.
Recent advances in management
CT enterography localises the transition point pre-operatively. Laparoscopic adhesiolysis, endoscopic stenting for obstructing colorectal cancers as a bridge to elective surgery, and enhanced-recovery protocols have shortened both hospital stay and the time to return of bowel function.
