Thyroid
The thyroid gland at the front of the neck can enlarge diffusely (goitre), form a single or multiple nodules, over- or under-secrete hormone, or — uncommonly — develop cancer. Each requires a distinct workup.
Symptoms
A visible swelling in the front of the neck, difficulty swallowing or hoarseness of voice, symptoms of an overactive thyroid (palpitations, weight loss, tremor) or an underactive one (fatigue, cold intolerance, weight gain).
Recent advances in management
Ultrasound with TI-RADS scoring and fine-needle aspiration guide the decision to operate. Hemithyroidectomy or total thyroidectomy is performed with routine nerve monitoring to protect voice; in selected small papillary cancers, radiofrequency ablation now offers a scarless alternative.
